Today, the findings, conclusions, and essential actions from the Independent Review of Maternity Services at Nottingham University Hospitals NHS Trust have been published. The review, led by Donna Ockenden and her team, examined maternity services at Nottingham University Hospitals NHS Trust.

It looked at 2,511 cases, including stillbirths, neonatal deaths, and serious injuries, and was commissioned after families raised longstanding concerns about the quality and safety of care and the way incidents were investigated.

Key information

The report sets out a series of recommended actions focused on improving how maternity services listen to women and families, strengthening staffing and training, standardising safety processes, and improving incident investigation and accountability.
