Kayleigh Griffiths is a 35-year-old mother of three, including Pippa Griffiths, who died avoidably at Shrewsbury and Telford Hospital NHS Trust, aged one day old.

The Trust attempted to exclude the family from an investigation into Pippa’s death, despite concerns being raised by Kayleigh on a number of occasions. 

This led Kayleigh to be concerned about what other events were being hidden at Shrewsbury and Telford Hospital NHS Trust, and contact was made with another bereaved mother, Rhiannon Davies. A bond was formed, and after months of searching for similar cases, a list of 23 cases where harm had occurred at the Trust was compiled.

The two families joined forces and wrote to the then Secretary of State for Health and Social Care, Jeremy Hunt, who agreed to commission the Ockenden Review, led by Baby Lifeline Honorary President Donna Ockenden. The initial report was published on 20 December 2020, and the final review was published on 30 March 2022. Both reports contain immediate and essential actions for trusts up and down the country. It is hoped the learning from the Ockenden Review will improve maternity safety across the country.

As a member of Baby Lifeline’s Family Voices Group, Kayleigh hopes to bring further ideas and experience to the charity.
